What is the macula? How does macular degeneration occur?

Macular degeneration, also known as age-related macular degeneration (AMD), is a common eye disease that occurs in individuals aged 50 and above. It is caused by the natural aging and deterioration of eye function. However, in modern times, prolonged use of electronic devices and excessive eye strain can lead to premature aging and degeneration of the eyes, resulting in macular degeneration even in younger individuals.

 The retina is essential for our vision, and the macula serves as not only the center of the retina but also the central visual hub. Any disease affecting the macula will inevitably impact vision, and in severe cases, it can lead to blindness.

In the early stages of macular degeneration

Patients may only experience slight dimness, similar to the difficulty in reading experienced with presbyopia. Gradually, the central vision becomes hazy and dark, resembling the appearance of cataracts. When the degenerationprogresses and forms new blood vessels, the images can become distorted, with straight lines appearing wavy. If the new blood vessels lead to bleeding or leakage, the affected area immediately turns into a dark shadow. Ultimately, individuals may experience a loss of reading ability in milder cases, while severe cases can result in blindness.

1.Visual Impairment : There will be a rapid or progressive decrease in vision. Acute cases often involve a significant decrease in vision in one eye, while progressive cases show a noticeable decline in vision over time.

2.Changes in Visual Field : The visual field is generally divided into central vision and peripheral vision. Macular degeneration mostly affects the central vision. Changes in central vision include the ability to clearly see a distinct circular, elliptical, or irregular dark spot against a white or light-colored background.

3.Color Vision Abnormalities : The macular region contains many color-sensitive cells called cones. Therefore, macular degeneration can cause abnormalities in color perception for individuals.

02 Lutein & Zeaxanthin

1.Zeaxanthin is an isomer of lutein and both belong to the carotenoid family, sharing the same chemical formula.

2.The carotenoids primarily absorbed by the human body are β-carotene, with lutein and zeaxanthin accounting for only about 20% and 3% respectively in the bloodstream. However, in the macula of the retina, lutein and zeaxanthin make up as much as 80% of the carotenoids present.

1.This ratio has been found to reduce the risk of age-related macular degeneration. While lutein and zeaxanthin concentrations in the bloodstream increase shortly after ingestion, the concentration in the macula does not respond as rapidly. Therefore, long-term and consistent accumulation is necessary for a meaningful increase in the macular concentration and to achieve eye protection.

2.Furthermore, the proportion of zeaxanthin to lutein intake is important because solely increasing zeaxanthin intake without considering the ratio will not have a beneficial effect on eye health.

How long should you take lutein?

What is the difference between Free Form Lutein and Esterified Lutein?

1.Free Form Lutein can be directly utilized by the human body without conversion and has a higher bioavailability compared to lutein supplements currently available in the market.

2.The first-generation lutein products were mostly in the form of esterified lutein, which has a larger molecular weight. During the digestion process in the human body, enzymes are required to break it down into smaller free form lutein for absorption. However, the conversion rate is less than 50%.

3.Therefore, the second-generation lutein products have developed special processing techniques to directly convert marigold extract into free form lutein that can be readily absorbed by the human body. Lutein exists in two forms: esterified lutein, found in orange-yellow fruits and egg yolks, and free form lutein, found in green vegetables.

The above diagram shows that esterified lutein has a larger molecular weight, nearly twice that of free form lutein. Therefore, it needs enzymes to break it down in the digestive tract before it can be absorbed and utilized by the human body. However, after this conversion process, less than 50% of lutein can be absorbed and utilized by the body.

According to the above figure shows, one study examined the response of serum lutein levels to free-form lutein and esterified lutein. The study involved 72 participants who were divided into two groups. Each group received a daily supplementation of 12mg of free-form lutein and 27mg of esterified lutein. After 7 days, the concentration of free-form lutein in the blood started to exceed that of esterified lutein. After 28 days, participants who received free-form lutein had a plasma lutein concentration that was 23% higher than those who received esterified lutein.
